It is unlawful for any medical personnel who provide treatment to a person or animal for an animal bite or injury caused by a pet animal or working dog to fail to report to the police department any information known regarding the animal bite or injury. The report shall be made within twenty four (24) hours after such information is received, and shall include with the report the name, address, and telephone number of the person making the report. The report may be made by telephone to the front desk of the police department, or may be left on the police department's voice mail or communicated to the police department by other electronic means.
